Recently, the team led by Professor Gan Feng from the Department of Cardiology, Beijing Aerospace General Hospital, successfully completed the first clinical application of the RattleSight Intravascular Ultrasound (IVUS) Catheter in China, bringing a brand-new option for precise diagnosis and treatment of Percutaneous CoronaryIntervention (PCI).
Equipped with a 20μm resolution and 60MHz frequency, the RattleSight intravascular ultrasound system delivers clinicians a clearer, more comprehensive and more convenient IVUS imaging experience.
Case Details:
Male, 46 years old
Chief Complaint: Intermittent chest pain for 2 years, aggravated for half a month
History of Present Illness: The patient had intermittent unprovoked retrosternal chest pain (resolved spontaneously within 1-2 minutes, aggravated after activity) two years ago, without standardized treatment due to intermittent recurrence. Half a month ago, the chest discomfort recurred with the same severity. Coronary angiography at the First Hospital of Shanxi Medical University revealed 80-90% stenosis at the left main coronary artery ostium, and coronary artery bypass grafting was recommended. The patient was then admitted to our hospital for further diagnosis and treatment with a diagnosis of "coronary heart disease with angina pectoris".
Past Medical History: History of hypertension, controlled with bisoprolol fumarate 2.5mg; history of hyperlipidemia.
Preoperative IVUS:
IVUS imaging shows a calcified stenotic lesion throughout the right coronary artery, with eccentric calcification at the ostium of the right coronary artery and a minimum lumen area of 4.78mm². There is 360° circular calcification in the middle segment, with a minimum lumen area of 2.1mm².
Treatment process: A 3.5mm×12mm Lepu Vasscrack IVL Balloon Catheter was used for intravascular lithotripsy therapy, with parameters of 4-6atm×10s for 8 cycles to treat the deep calcified lesion of the right coronary artery. A stent was implanted after using lithotripsy therapy.
Postoperative IVUS: Postoperative IVUS shows good stent apposition and a significant increase in lumen area.
